Please do the following when filling out the SCMWD Water Tap Application form:

  1. CLICK the page of the application below.
  2. SAVE the page to your desktop computer/tablet.
  3. Open using Adobe Reader. Click the Adobe Logo on the immediate left of this website under Consumer Confidence Reports if you do NOT have the latest version of Adobe Reader to download on your desktop/tablet (it's FREE)
  4. After opening the SCMWD Water Tap Application form on Adobe Reader, Place the cursor at first box and begin filling out application. SAVE page after you have finished filling out the application.
  5. Print from your printer to bring into the District office.
